1 The Injury Master system allows you to create the Partial Incapacity Form (reimbursement request) for any wage compensation where the employer is entitled to reimbursement from WorkCover QLD. Once created, use the form to:

3 *. Email to the agent/insurer or any other relevant party directly from the system
4 *. Print off - whether to post, fax or file for you records
5 *. Save to you documents/system

7 //**Please note** unlike other states/territories a reimbursement request form is not required where the worker is **fully off work**. //

9 Once a period has been set in the system (where wage compensation has been paid which us eligible for reimbursement) a tick box will appear next to the relevant week. By ticking this box a button will appear giving you the option to **Submit Partial Incapacity**, as shown below.

13 A pop-up will then appear confirming if you want to proceed to submit reimbursement request, select the blue **Generate** button to proceed.

17 A medical certificate should be uploaded to cover the period of incapacity (unless not required). If the medical certificate for the period is not uploaded into the system, the pop up will contain an alert (select **Generate **to continue).

19 You will then be redirected to the Partial Incapacity document in the Generate Documents area.

23 Some of the information already in the system will be automatically captured in the relevant fields, such as the workers details and employer contact details. This information can be edited if required.

27 The //daily breakdown of hours// **must** be complete. \\//Complete all the required fields to ensure the form is accepted by the agent/insurer. //

29 Once all relevant details have been entered select either the **Complete** button to generate the final document or the **Save Changes** button to save the progress so far.

33 Once generated the form can be downloaded or amended. View the document by selecting **//Download//** or edit the form by selecting the **//Edit//** button.

50 If the worker has no capacity for work you will still need to provide the medical certificate to WorkCover QLD for the period (but no form). You can send the medical certificate to WorkCover QLD directly from Injury Master by going to the Documents tab and selecting the S//hare via Email //button next to the relevant medical certificate, as shown below.
